    The School shall exercise responsible care to protect all property. However, it is understood that Maplebrook does not assume financial responsibility for valuables, personal clothing, property, or belongings of the applicant.I hearby apply for a place at Maplebrook School for my youngster and understand and accept all rules, conditions, and requirements of the school. The tuition contract is for the entire school year, or, in case of students applying after the opening of the school year, for the remainder of the year. Further, I attest to the fact that the information furnished on this Application for Admission form, together with all information and materials gathered and released to Maplebrook School, are accurate and a true representation of my youngster.

    How to Apply

    Complete the application and email the information listed below to: admissions@maplebrookschool.org.

    • Latest psycho-educational battery (within 3 years), including the WISC/WAIS
    • Academic achievement data (Woodcock-Johnson, KTEA, WIAT, etc.) including grade equivalents]
    • Transcript
    • Medical/social reports containing information necessary for your child/student’s well-being.

    Other pertinent information may be requested by the Admissions Office. Once the application is received and the above materials are reviewed, a visit to the school and interview will be scheduled.

    The Admissions Office will contact you to set up an appointment. Visits are scheduled through the Admissions Office and include a student-guided tour of the campus.
